Format: 4 items in total: frequency of urinary incontinence, amount of leakage, overall impact of urinary incontinence & self-diagnostic.

Scoring: 3 items (frequency of urinary incontinence, amount of leakage, overall impact of urinary incontinence) are summed to provide a score between 0 & 21. Higher scores reflect increased urinary incontinence and poorer QoL.
